Incident 7: Timeout regression
Scenario 3, run 1 of 3, phase 1, 30 Sep 2026. Run 03-timeout-regression-20260930T205355Z at commit 5085754.
A configuration deploy of fulfillment lowers its payment call timeout from 3 s to 50 ms. The payment provider is as fast as ever (about 60 ms), but fulfillment now gives up on almost every call. Unlike scenario 4, the provider did not change: the caller did, and the change is a recorded deploy of fulfillment.

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
payments / slow dependency, confidence 95. wrong
The payments service experienced high latency with p99 duration near 3.8 seconds, causing calls from the fulfillment service to fail and triggering the payment failures alarm.
Hypotheses at the end
- likely slow_dependency (payments service is timing out when fulfillment calls it)
- ruled out bad_deploy
- ruled out config_regression

Summary
The payments service experienced high latency with p99 duration near 3.8 seconds, causing calls from the fulfillment service to fail and triggering the payment failures alarm.
Impact
The page: nightshift-payment-failures was ALARM (since 2026-09-30T21:48:29Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [49.0 (30/09/26 21:47:00)] was greater than or equal to the threshold (3.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

Proposed fix (for a human to decide; the agent changed nothing)
- Investigate why payments service duration is high (p99 ~3.8s) and causing payment failures in fulfillment.
Hypotheses
- likely: slow_dependency (payments service is timing out when fulfillment calls it)
- ruled_out: bad_deploy
- ruled_out: config_regression
What the investigation cost
- 4 model calls, 14,438 input and 462 output tokens (14,900 total)
- 11 steps, 9 s of wall clock
- 1,093,298 bytes of logs scanned
- 0 replies rejected by the provider for bad tool arguments
fulfillment / config regression, confidence 95. wrong
The fulfillment service's PAYMENT_TIMEOUT_SECONDS environment variable was set to 0.05 seconds, causing all calls to the payments service to fail due to timeout. This change occurred during a deploy at 21:47:11 and directly matches the spike in payment failures.
Hypotheses at the end
- likely config_regression: PAYMENT_TIMEOUT_SECONDS was lowered, causing fulfillment to fail when calling payments
- ruled out slow_dependency: the payments service is slow, causing fulfillment to timeout
- ruled out throttling: the payments service is throttling fulfillment calls
- ruled out bad_deploy: the deploy did not change code but only a misconfigured timeout setting

Check the **environment variables** for the `fulfillment` service to confirm if `PAYMENT_TIMEOUT_SECONDS` was recently changed (Step 9). 2. Check the **X-Ray traces** for the `fulfillment` service to see if there are throttling errors or slow responses from the `payments` service (Step 10). 3. Confirm the **state of the `payments` service** by checking its recent errors or latency (Step 11). This will help determine whether the issue is due to a timeout regression, throttling, or a slow dependency.

This is an extremely low value and would cause almost all calls to the `payments` service to fail due to timeout, even if the `payments` service is otherwise healthy. - The alarm for `PaymentFailures` in the `fulfillment` service spiked at **21:47:00** (Step 4), coinciding with the deploy at **21:47:11** (Step 5), which changed this setting. - The X-Ray traces for `fulfillment` show no throttling or errors (Step 10), but the traces are limited to **83 samples** and do not directly show timeout failures. However, the logs (Step 7) confirm failures with a timestamp matching the alarm. 2. **Slow Dependency (Ruled Out)** - The `payments` service's **p99 duration is ~61 seconds** (Step 11), which is high but not directly causing the failures in `fulfillment`. Summary
The fulfillment service's PAYMENT_TIMEOUT_SECONDS environment variable was set to 0.05 seconds, causing all calls to the payments service to fail due to timeout. This change occurred during a deploy at 21:47:11 and directly matches the spike in payment failures.

- Revert the PAYMENT_TIMEOUT_SECONDS environment variable to a valid value (e.g., 10 seconds) for the fulfillment service.
