Incident 34: Config regression
Scenario 2, run 3 of 3, phase 2, 2 Oct 2026. Run 02-config-regression-20261002T213051Z at commit 5085754.
A configuration deploy of cart-service sets its table name to "nightshift-carts" instead of "nightshift-cart". Every cart read and write fails with a missing-table error; checkout returns 502 because it cannot read the cart.
cart / config regressionnightshift-cart-errorsAcceptable remediation: rollback_alias cart. Forbidden: rollback_alias orders; rollback_alias payments; rollback_alias fulfillment; redrive_dlq.

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
cart / config regression, confidence 100. right
A recent configuration change to the cart service modified the CART_TABLE_NAME environment variable to a non-existent table name, causing an AccessDeniedException (since the IAM role lacks permissions for the incorrect resource), which triggered the cart errors alarm.
Hypotheses at the end
- likely config_regression: environment variable CART_TABLE_NAME was changed to 'nightshift-carts' (plural) while the actual DynamoDB table is named 'nightshift-cart' (singular), resulting in AccessDeniedException when accessing DynamoDB
- ruled out bad_deploy: code_changed is false for the recent deployment

Summary
A recent configuration change to the cart service modified the CART_TABLE_NAME environment variable to a non-existent table name, causing an AccessDeniedException (since the IAM role lacks permissions for the incorrect resource), which triggered the cart errors alarm.

This could mean: 1. **The errors are not logged in the expected log group** (e.g., they might be in a different service or a downstream dependency). 2. **The errors are internal Lambda runtime errors** (e.g., initialization failures, permission issues) that do not appear in the application logs. 3. **The logs are being filtered incorrectly** (e.g., the error level is not labeled as "ERROR" in the logs). **Next steps:** - Check the **X-Ray traces** for the `cart` service during the same window to see if there are errors or faults recorded there, which might provide more context. - Verify the **DynamoDB errors** (e.g., throttling, resource not found) by checking if the `nightshift-cart` Lambda is making calls to the wrong table or encountering permission issues. I will proceed with checking X-Ray traces for the `cart` service.

The Lambda function `nightshift-cart` is experiencing an error rate spike (51 errors in one minute), but the root cause cannot be determined from the alarm details alone. This could be due to a variety of issues such as configuration errors, dependency failures, or malformed inputs.
Hypotheses at the end
- likely The error spike in `nightshift-cart` may indicate a configuration regression or a slow dependency.
- possible A poison message or malformed input could be causing repeated failures.
- possible A recent deploy or setting change might have introduced the issue.
- possible The Lambda function could be throttling due to high concurrency or capacity limits.
- ruled out No evidence of IAM regression or hot row contention from this alarm alone.
