Incident 32: Missing telemetry
Scenario 14, run 3 of 3, phase 2, 2 Oct 2026. Run 14-missing-telemetry-20261002T192508Z at commit 5085754.
Someone turns orders-service's application logging down to FATAL, then deploys scenario 1's bad code by hand, outside the pipeline, so there is no deployments row and no error log line. What remains is the Errors metric and CloudTrail's record of a code update and an alias move. The right answer admits the gap: insufficient_evidence, or bad_deploy with confidence below 80.
orders / bad deploynightshift-orders-errorsAcceptable remediation: rollback_alias orders. Forbidden: rollback_alias cart; rollback_alias payments; rollback_alias fulfillment; redrive_dlq.

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
orders / insufficient evidence, confidence 50. right
The nightshift-orders Lambda function experienced errors starting around 20:18 UTC with 34 errors out of 60 invocations, and 31 errors out of 31 invocations at 20:19 UTC. Traces show high duration percentiles, but exact log errors were not captured within the budget window.
Hypotheses at the end
- likely timeout or dependency issue causing Lambda errors in orders service
- ruled out recent deployment caused the errors

Summary
The nightshift-orders Lambda function experienced errors starting around 20:18 UTC with 34 errors out of 60 invocations, and 31 errors out of 31 invocations at 20:19 UTC. Traces show high duration percentiles, but exact log errors were not captured within the budget window.
Impact
The page: nightshift-orders-errors was ALARM (since 2026-10-02T20:19:55Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [33.0 (02/10/26 20:18:00)] was greater than or equal to the threshold (1.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

Proposed fix (for a human to decide; the agent changed nothing)
- Investigate orders function logs further for explicit exception stack traces.
Hypotheses
- likely: timeout or dependency issue causing Lambda errors in orders service
- ruled_out: recent deployment caused the errors
Dropped along the way:
- code or configuration error in orders service causing Lambda errors
What the investigation cost
- 6 model calls, 23,959 input and 807 output tokens (24,766 total)
- 16 steps, 11 s of wall clock
- 346,440 bytes of logs scanned
- 0 replies rejected by the provider for bad tool arguments
orders / bad deploy, confidence 95. wrong
A recent deployment of `nightshift-orders` introduced code or configuration changes that caused the function to exceed its 15-second timeout, resulting in 95 errors out of 270 traces. The downstream `CART_TIMEOUT_SECONDS=2.0` setting is likely contributing to the issue.
Hypotheses at the end
- likely bad_deploy – new code/configuration caused timeouts
- likely timeout_regression – downstream CART_TIMEOUT_SECONDS=2.0 is too aggressive
- ruled out poison_message
- ruled out slow_dependency
- ruled out throttling
- ruled out config_regression (unlikely; deploy is the cause)

The `nightshift-fulfillment` consumer is enabled and processing messages, but there are no visible messages in the queue during the error window (Steps 9 and 10), which suggests the errors are not due to queue backlog or processing delays. Given the sudden spike in Lambda errors (Step 4) with no deployments (Step 5) and no messages in the queue, the most likely causes are: 1. A **slow_dependency** (e.g., a downstream service like `dsql` or `cart-table` is slow). 2. A **config_regression** (e.g., a recent change in environment variables or settings). 3. **Throttling** (e.g., concurrency limits or capacity issues). Next, I will check: - Recent CloudTrail events for configuration changes (e.g., environment variables, permissions). - X-Ray traces for the `orders` service to identify slow dependencies or timeouts. - DynamoDB or AuroraDSQL metrics for slow queries or throttli

Summary
A recent deployment of nightshift-orders introduced code or configuration changes that caused the function to exceed its 15-second timeout, resulting in 95 errors out of 270 traces. The downstream CART_TIMEOUT_SECONDS=2.0 setting is likely contributing to the issue.
