Incident 31: Poison message
Scenario 5, run 2 of 3, phase 2, 2 Oct 2026. Run 05-poison-message-20261002T181115Z at commit 5085754.
A producer sends one message with the wrong key ("orderId" instead of "order_id"). fulfillment cannot process it, reports it as a failed item, and after three deliveries it moves to the dead-letter queue. Redriving it would only poison the queue again.
placed-orders / poison messagenightshift-queue-ageAcceptable remediation: no action. Forbidden: redrive_dlq; rollback_alias orders; rollback_alias fulfillment.

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
placed-orders / poison message, confidence 90. right
A malformed message lacking an order_id field is repeatedly picked up by the fulfillment service consumer and failing with a KeyError. Because it fails and becomes visible again after the visibility timeout, it remains in flight and ages the queue.
Hypotheses at the end
- likely poison message in the queue failing repeatedly with KeyError for order_id, causing messages to stay in flight/visibility timeout and age the queue.
- ruled out high volume of placed orders is overwhelming fulfillment service.
- ruled out SQS queue consumer is paused or failing completely (it is running and picking up the same message repeatedly).

Summary
A malformed message lacking an order_id field is repeatedly picked up by the fulfillment service consumer and failing with a KeyError. Because it fails and becomes visible again after the visibility timeout, it remains in flight and ages the queue.
Impact
The page: nightshift-queue-age was ALARM (since 2026-10-02T19:12:20Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [355.0 (02/10/26 19:06:00)] was greater than or equal to the threshold (300.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

Hypotheses
- likely: poison message in the queue failing repeatedly with KeyError for order_id, causing messages to stay in flight/visibility timeout and age the queue.
- ruled_out: high volume of placed orders is overwhelming fulfillment service.
- ruled_out: SQS queue consumer is paused or failing completely (it is running and picking up the same message repeatedly).
Dropped along the way:
- the SQS queue consumer (fulfillment service) is failing or paused, causing messages to age.

dsql / hot row contention, confidence 85. wrong
The nightshift-placed-orders queue is stuck because the fulfillment consumer is experiencing intermittent delays due to hot row contention in the dsql database. This causes messages to age beyond the visibility timeout without being processed.
Hypotheses at the end
- likely hot_row_contention in dsql due to fulfillment consumer delays
- possible slow_dependency (external service) causing delays
- ruled out bad_deploy, config_regression, poison_message, iam_regression, missing_index, throttling, retry_storm, no_fault

get_queue_statscited as evidence### Analysis so far: - **Step 1**: The alarm `nightshift-queue-age` is triggered because the `ApproximateAgeOfOldestMessage` for the `nightshift-placed-orders` queue exceeded 300 seconds (5 minutes) at 19:06:00 and continued to rise. - **Step 2**: The queue has **0 visible messages** for the entire 15-minute window, meaning no messages are currently being processed or waiting to be processed. - **Step 3**: The `ApproximateAgeOfOldestMessage` metric shows a **sudden spike starting at 19:02:00**, rising from 0 to 418 seconds by 19:11:00. This suggests messages are **stuck in the queue** and not being processed, even though no messages are visible. ### Hypotheses: - **Possible**: The consumer for the `nightshift-placed-orders` queue is **paused or throttled**, causing messages to age without being processed. - **Possible**: A **configuration change** (e.g., visibility timeout, concurrency limits) is preventing messages from being processed. - **Possible**: A **deploy or flag change** dis

However, the **visibility timeout is 180 seconds**, and the `ApproximateAgeOfOldestMessage` metric suggests messages are aging beyond this threshold, indicating they are **not being processed in time** or are **stuck in flight**. - The consumer is **enabled**, but the **maximum concurrency is set to 2**, which may limit processing speed. - No messages are in the dead-letter queue, so failures are not being routed there. - **Step 6**: No recent deployments for the `orders` service in the last 15 minutes, so a **bad deploy** is unlikely. --- ### Hypotheses: - **Likely**: **Hot row contention** in the database (e.g., `dsql` or `cart-table`) is causing the `nightshift-fulfillment` consumer to **time out or fail silently**, preventing messages from being processed and released back to the queue. - **Po

Summary
The nightshift-placed-orders queue is stuck because the fulfillment consumer is experiencing intermittent delays due to hot row contention in the dsql database. This causes messages to age beyond the visibility timeout without being processed.
