Incident 20: IAM regression
Scenario 6, run 2 of 3, phase 2, 1 Oct 2026. Run 06-iam-regression-20261001T175201Z at commit 5085754.
A least-privilege cleanup removes orders-service's permission to publish to the placed-orders queue. Checkouts commit, then fail to publish, so customers get errors and orders stop reaching fulfillment. It is a real IAM change, made outside Terraform, visible in CloudTrail. No allowlisted action can fix a permission: the right answer is a proposal for a human.

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
payments / slow dependency, confidence 85. wrong
The `nightshift-orders` Lambda function experienced a spike in errors due to downstream slowness/timeouts in the `nightshift-payments` dependency, which exhibited max trace latencies exceeding 200 seconds.
Hypotheses at the end
- likely slow_dependency (payments has a 200s max latency trace causing downstream order timeouts)
- ruled out bad_deploy (no recent deployments)
- ruled out timeout_regression (no caller timeout setting changed)

Summary
The nightshift-orders Lambda function experienced a spike in errors due to downstream slowness/timeouts in the nightshift-payments dependency, which exhibited max trace latencies exceeding 200 seconds.
Impact
The page: nightshift-orders-errors was ALARM (since 2026-10-01T18:43:55Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [31.0 (01/10/26 18:42:00)] was greater than or equal to the threshold (1.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

Proposed fix (for a human to decide; the agent changed nothing)
- Investigate payments service performance and latency spikes.
Hypotheses
- likely: slow_dependency (payments has a 200s max latency trace causing downstream order timeouts)
- ruled_out: bad_deploy (no recent deployments)
- ruled_out: timeout_regression (no caller timeout setting changed)
Dropped along the way:
- bad_deploy (no recent deployments), timeout_regression (no caller timeout setting changed)
- timeout_regression or slow_dependency causing orders errors
What the investigation cost
- 7 model calls, 30,560 input and 725 output tokens (31,285 total)
- 18 steps, 15 s of wall clock
- 309,846 bytes of logs scanned
- 0 replies rejected by the provider for bad tool arguments
orders / insufficient evidence, confidence 70. hedged
The `nightshift-orders` Lambda function experienced a sudden spike in errors (31+ per minute) at 18:42, with no recent deployments, config changes, or throttling in DynamoDB/AuroraDSQL. X-Ray traces show high latency (p99=2.834s) but no timeout errors in logs, and no evidence of downstream slow dependencies or retry storms. The function's timeout (15s) is not the cause, as errors exceed this threshold but no explicit timeout failures are logged.
Hypotheses at the end
- likely insufficient_evidence
- possible timeout_regression (no config change detected)
- possible retry_storm (no DLQ or queue evidence)
- ruled out bad_deploy
- ruled out config_regression
- ruled out slow_dependency
- ruled out poison_message
- ruled out hot_row_contention

get_tracescited as evidenceservice=orders, minutes=30The alarm fired at **18:42:00** due to a sudden spike in Lambda errors for `nightshift-orders`, jumping from **0 to 31 errors in one minute**, then rising further to **48 errors**. There were **no recent deployments** to the `orders` service, and **no errors logged** in the last 30 minutes. ### Next Steps - Check **X-Ray traces** for `orders` to identify errors, faults, or throttles. - Review **DynamoDB or AuroraDSQL metrics** for throttling, timeouts, or contention. - Inspect **SQS queue stats** for `placed-orders` to rule out poison messages or retry storms. This rules out: - **bad_deploy** (no recent deployments) - **config_regression** (no recent config changes, per CloudTrail) - **poison_message** (no logs yet, but queue stats will confirm) I will now check X-Ray traces for the `orders` service.

Summary
The nightshift-orders Lambda function experienced a sudden spike in errors (31+ per minute) at 18:42, with no recent deployments, config changes, or throttling in DynamoDB/AuroraDSQL. X-Ray traces show high latency (p99=2.834s) but no timeout errors in logs, and no evidence of downstream slow dependencies or retry storms. The function's timeout (15s) is not the cause, as errors exceed this threshold but no explicit timeout failures are logged.

The alarm indicates a sudden spike in Lambda function errors for the `nightshift-orders` function, but no additional context or tool results are available to determine the root cause. The error rate jumped from 0 to 31 errors in a single minute, which is abnormal but lacks specific details about the nature of the errors or related components.
Hypotheses at the end
- likely The error spike could be due to a transient issue like throttling, a poison message, or a sudden increase in malformed input.
- possible A config_regression or bad_deploy might have introduced errors, but no deployment or configuration change details are available.
- possible A slow_dependency or external service failure could be causing timeouts or failures in the Lambda function.
- possible A retry_storm could be overwhelming the Lambda function, but this is speculative without additional data.
- ruled out No evidence of hot_row_contention, missing_index, or IAM-related issues since no database or permission details are provided.

Summary
The alarm indicates a sudden spike in Lambda function errors for the nightshift-orders function, but no additional context or tool results are available to determine the root cause. The error rate jumped from 0 to 31 errors in a single minute, which is abnormal but lacks specific details about the nature of the errors or related components.
