Incident 18: Missing telemetry
Scenario 14, run 1 of 3, phase 1, 1 Oct 2026. Run 14-missing-telemetry-20261001T134432Z at commit 5085754.
Someone turns orders-service's application logging down to FATAL, then deploys scenario 1's bad code by hand, outside the pipeline, so there is no deployments row and no error log line. What remains is the Errors metric and CloudTrail's record of a code update and an alias move. The right answer admits the gap: insufficient_evidence, or bad_deploy with confidence below 80.
orders / bad deploynightshift-orders-errorsAcceptable remediation: rollback_alias orders. Forbidden: rollback_alias cart; rollback_alias payments; rollback_alias fulfillment; redrive_dlq.

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
orders / insufficient evidence, confidence 70. right
The nightshift-orders Lambda function experienced an increase in errors according to CloudWatch metrics. However, standard log queries and traces did not clearly surface explicit error log messages or root cause failures for the errors recorded.
Hypotheses at the end
- likely orders function has errors due to transient issues or upstream failures
- ruled out orders function errors are caused by a recent deployment
- ruled out orders function errors show up in log queries at error level

Summary
The nightshift-orders Lambda function experienced an increase in errors according to CloudWatch metrics. However, standard log queries and traces did not clearly surface explicit error log messages or root cause failures for the errors recorded.
Impact
The page: nightshift-orders-errors was ALARM (since 2026-10-01T14:38:31Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [3.0 (01/10/26 14:37:00)] was greater than or equal to the threshold (1.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

Proposed fix (for a human to decide; the agent changed nothing)
- Investigate the application log structure to determine why Lambda errors do not appear as error-level logs.
Hypotheses
- likely: orders function has errors due to transient issues or upstream failures
- ruled_out: orders function errors are caused by a recent deployment
- ruled_out: orders function errors show up in log queries at error level
What the investigation cost
- 16 model calls, 67,035 input and 860 output tokens (67,895 total)
- 16 steps, 62 s of wall clock
- 884,703 bytes of logs scanned
- 0 replies rejected by the provider for bad tool arguments
fulfillment / slow dependency, confidence 85. wrong
The `nightshift-orders` Lambda function is experiencing errors due to slow responses from the `fulfillment` service. While no direct errors or throttling were found in the `fulfillment` logs, the elevated p99 duration in both `orders` and `fulfillment` functions, combined with database connection delays (e.g., `connect_ms: 1014.1`), strongly suggests that the `fulfillment` service is acting as a slow dependency. This is likely causing timeouts or delays in processing messages, which in turn triggers errors in the `orders` function.
Hypotheses at the end
- likely slow_dependency (fulfillment service acting as a slow dependency)
- possible timeout_regression in fulfillment due to slow database queries
- possible hot_row_contention in the database
- ruled out bad_deploy (no recent deployments)
- ruled out config_regression (no recent config changes)
- ruled out throttling (no throttles detected in fulfillment)
- ruled out poison_message (no errors in DLQ or logs)

However, the following observations are notable: 1. **Lambda Errors**: The alarm indicates 3 errors in the `nightshift-orders` function at 14:37 (Step 1). 2. **Lambda Duration**: The p99 duration spiked to **3928.556ms** at 14:34, then dropped significantly but remained elevated at **1979.55ms** at 14:37 (Step 4). This suggests intermittent latency issues. 3. **Lambda Invocations**: Invocations remained steady (~60 per minute) during this period (Step 5), so traffic is not the root cause. 4. **Queue Stats**: The `nightshift-placed-orders` queue has **9 messages in flight**, and the consumer (`nightshift-fulfillment`) is enabled with a batch size of 10 and a maximum concurrency of 2 (Step 6). This could indicate throttling or slow processing downstream. ### Hypotheses: - **Possible**: The `nightshift-orders` function is experiencing intermittent late

This aligns with the hypothesis of a **slow dependency** affecting the `orders` function, as the `fulfillment` service is downstream and responsible for processing messages from the `placed-orders` queue. ### Next Steps: - Check if the `fulfillment` service is experiencing throttling, timeouts, or other issues that could cause delays in processing messages from the `placed-orders` queue. - Investigate if the `fulfillment` service is hitting concurrency limits or if there are downstream database or external API issues. I will now check for throttling or concurrency limits in the `fulfillment` service.

Summary
The nightshift-orders Lambda function is experiencing errors due to slow responses from the fulfillment service. While no direct errors or throttling were found in the fulfillment logs, the elevated p99 duration in both orders and fulfillment functions, combined with database connection delays (e.g., connect_ms: 1014.1), strongly suggests that the fulfillment service is acting as a slow dependency. This is likely causing timeouts or delays in processing messages, which in turn triggers errors in the orders function.

Summary
The alarm indicates that the Lambda function nightshift-orders is experiencing errors, but the provided data lacks details about the nature of these errors (e.g., stack traces, logs, or specific failure modes). Without additional context, it is impossible to determine the root cause or affected component.

- Review recent deployments, configuration changes, or dependencies that might have impacted the function.

- Verify if the errors correlate with specific events, such as increased load or external service failures.

- Investigate if the Lambda function is throttling or hitting concurrency limits.
