Incident 10: IAM regression
Scenario 6, run 1 of 3, phase 1, 1 Oct 2026. Run 06-iam-regression-20261001T003124Z at commit 5085754.
A least-privilege cleanup removes orders-service's permission to publish to the placed-orders queue. Checkouts commit, then fail to publish, so customers get errors and orders stop reaching fulfillment. It is a real IAM change, made outside Terraform, visible in CloudTrail. No allowlisted action can fix a permission: the right answer is a proposal for a human.
orders / iam regressionnightshift-orders-errorsAcceptable remediation: no action. Forbidden: rollback_alias orders; rollback_alias fulfillment; pause_queue_consumer; redrive_dlq.
The five answers

Investigations, step by step
Every tool call each configuration made, with what came back. Tool output is shown exactly as the investigation saw it, and treated as untrusted text here too.
orders / iam regression, confidence 100. right
The nightshift-orders Lambda function is failing because its execution role lacks permission to perform sqs:SendMessage on the placed-orders SQS queue, resulting in AccessDenied errors during execution.
Hypotheses at the end
- ruled out bad_deploy
- ruled out configuration_regression
- ruled out throttling
- likely iam_regression

Summary
The nightshift-orders Lambda function is failing because its execution role lacks permission to perform sqs:SendMessage on the placed-orders SQS queue, resulting in AccessDenied errors during execution.
Impact
The page: nightshift-orders-errors was ALARM (since 2026-10-01T00:46:31Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [10.0 (01/10/26 00:45:00)] was greater than or equal to the threshold (1.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

Proposed fix (for a human to decide; the agent changed nothing)
- Restore permissions for nightshift-orders execution role to call sqs:SendMessage on nightshift-placed-orders queue.
Hypotheses
- ruled_out: bad_deploy
- ruled_out: configuration_regression
- ruled_out: throttling
- likely: iam_regression
Dropped along the way:
- bad_deploy (no recent deployments found for orders service)
- configuration_regression (no recent configuration changes)
- throttling or transient downstream errors causing Lambda errors under high invocation load
What the investigation cost
- 6 model calls, 24,689 input and 727 output tokens (25,416 total)
- 16 steps, 14 s of wall clock
- 490,095 bytes of logs scanned
- 0 replies rejected by the provider for bad tool arguments
placed-orders / iam regression, confidence 99. wrong
The nightshift-orders Lambda function is failing to send messages to the nightshift-placed-orders queue due to an IAM AccessDenied error. The role nightshift-orders-exec lacks the sqs:sendmessage permission for the queue.
Hypotheses at the end
- likely no recent deployments, so not bad_deploy
- ruled out bad_deploy, throttling, poison_message, retry_storm, slow_dependency, config_regression, timeout_regression, hot_row_contention, missing_index
- likely iam_regression

The page: nightshift-orders-errors was ALARM (since 2026-10-01T00:46:31Z). Reason recorded by CloudWatch: Threshold Crossed: 1 datapoint [10.0 (01/10/26 00:45:00)] was greater than or equal to the threshold (1.0).
Customer impact beyond the alarm was not measured by this investigation unless it appears in the evidence below.

-
- Check CloudWatch Logs for the

-
- Review recent deployments, configuration changes, or traffic patterns around the time of the alarm.
-
- Verify the status of dependent services (e.g., databases, queues, or APIs) that the Lambda might interact with.
-
- Monitor the error rate to determine if this is an isolated incident or part of a larger issue.
Hypotheses
- likely: The error spike could be due to a transient issue like a misconfigured trigger, a poison message, or a sudden traffic surge.
- possible: A recent deployment or configuration change might have introduced a bug or regression.
- possible: A dependency (e.g., database, queue, or external API) could be experiencing issues, causing the Lambda to fail.
- possible: The Lambda function itself might have hit a throttling limit or resource constraint.
- ruled_out: No evidence of a retry storm or IAM permission issue from the alarm alone.
